Covid Vaccine Consent Form

We have been notified that the school immunisation team will be at Charlton School on Monday 18th and Tuesday 19th October 2021 to offer a vaccination to all students aged 12 to 16 who have been given consent to have the vaccination.

Dear Parent/Carer,

We have been notified that the school immunisation team will be at Charlton School on Monday 18^th and Tuesday 19^th October 2021 to offer a vaccination to all students aged 12 to 16 who have been given consent to have the vaccination.

Please find attached a letter from the NHS vaccination team, including a set of FAQs. Only children aged 12 and above can have the vaccine and students who have had Covid-19 must wait 28 days from their positive PCR test prior to having a vaccine.

Students will be issued with a paper copy of the consent form on Friday, we have also attached a copy here. The consent forms need to be returned to reception by 9:00am on Thursday 14^th October.

Please can students either wear a short-sleeved shirt on the day of their vaccination or a t-shirt under their shirt to assist the vaccinators to have easy access to their upper arm.

Advanced notice – we will also be facilitating Flu Vaccinations for Years 7 to 11 on Friday 22^nd October, further details and a consent form will follow at the end of next week.
